There are vaccines for Hep A and Hep B. None for Hep C yet. Nor Hep V for the True Blood fans. Twinrix is the name of a combined Hep A & B vaccine (2 doses) - your health insurance most likely pays for it.

Hep A outbreaks are relatively frequent at restaurants (in my state).

Every state in the US has an immunization registry (they're all hosted by WebIZ) so they all look the same and produce the same sort of document. Checking states I've lived in, some of the records go back to 2008. My insurance pays for all vaccines except for "travel vaccines".
